Application

Five Minutes. Read by a Human.

Every application is reviewed personally within 24 hours. If you’re a fit, your lab order goes out that day. If you’re not, we’ll tell you why — before taking any payment.

Before You Start

What You’ll Need.

01

Basic contact info

Name, email, phone, and your U.S. state of residence. Our affiliated physician network is licensed across all 50 states.

02

General health info

Any major medical conditions and current medications. Be honest — the physician uses this to confirm the program is safe for you.

03

Your symptoms

A short, honest read on what feels off. Energy, drive, recovery, sleep, mood. No need to be clinical — plain language is fine.

04

Five minutes of focus

The form is short but the questions are real. Give it five minutes when you won’t be interrupted.

Start Your Application.

What happens next: a care coordinator reads your submission within 24 hours. If you’re a fit, you’ll be invited to schedule the intake consult — your first charge happens after you confirm. If you’re not a fit, we’ll tell you honestly — before any payment.

By submitting, you confirm that you are 18 years of age or older and consent to be contacted about your eligibility for the RestoreLab Restoration Program. Treatment decisions are made solely by a licensed prescribing clinician exercising independent medical judgment. Completing this form does not guarantee qualification for treatment. All information is HIPAA-protected. RestoreLab does not share, sell, or market based on your data. [Counsel-reviewed TRT INFORMED CONSENT placeholder — DO NOT LAUNCH WITHOUT REPLACING. The affiliated medical practice must obtain documented informed consent for testosterone therapy before prescribing. Standard items typically covered: cardiovascular considerations, polycythemia/erythrocytosis monitoring, fertility/spermatogenesis suppression, prostate/PSA monitoring, sleep-apnea exacerbation, mood changes, thromboembolic risk, and estradiol-related effects. Operator: replace this placeholder with the practice’s counsel-reviewed informed-consent text or link to a separate consent document patients sign during the intake call.]

After You Apply

What Happens, and When.

01

Within 24 hours

A care coordinator reads your application personally. You hear back by email or phone — whether accepted or not. Declines come with a brief honest explanation.

02

Within 3 business days

If accepted, you’re scheduled for a non-clinical onboarding call and issued a lab order for a LabCorp or Quest near you. No charges until the onboarding call.

03

Within 7–10 days

Baseline labs complete, physician consult done, protocol designed, custom-compounded medication shipped to your door in unmarked packaging. You’re on.

One more thing

Applying Is Not a Commitment.

No payment is taken during the application. If you’re accepted, you’ll have a full onboarding call to ask questions and confirm the cost before any charge — you’re choosing whether to keep going at every step.
